Reporting Incidents/Accidents: Guideline Procedures

Following any unsavoury incident before, during or after a match or training a member of the management team (Inc parental representative) should first make a verbal report to the Secretary.
The next stage is to provide the Secretary with a written copy of a report detailing the incident – * included should be such detail as the age group of the team – the match details (name of the opposition/home or away – venue), name of referee, kick off time, name of the person(s) involved in the incident and their role (i.e.) player, manager, parent, opponent etc. Then a detailed report of the incident. The report should be signed and dated *. The Secretary shall acknowledge all such reports.
If the matter should be directed to the CWO, -it should be done immediately by phone in the first instance and a written report to follow within 24 hours. This report should contain the information as listed above in paragraph two*. A courtesy would be to inform the Secretary that this has happened for record purposed – the Secretary shall then confirm the report has reached the chairperson.
If the incident is referred to the CWO the matter shall be dealt with by the CWO and the Chairperson as per FAI directives – the Chairman shall keep the persons involved up to date as per directives.
If the incident requires internal disciplinary action, an investigation shall be conducted and the appropriate action taken. This will be in accordance to our code of conduct/constitution as it applies at the time. Normal guidelines of natural justice shall apply.
If an accident has occurred the direction as per paragraph two* shall be observed and additional information regarding doctors details, hospital involved etc shall also be included.
